Introduction to the Form Module

The Form Module allows site editors to build, launch, and manage custom forms using standard drag-and-drop tools.

Security & Privacy Compliance Warning

To maintain compliance and protect user privacy, DO NOT use the Form Module to collect sensitive Personally Identifiable Information (PII) or confidential data, such as:

  • Social Security Numbers (SSN)
  • Student ID numbers (RedID)
  • Credit card, bank, or payment details
  • Driver's license or government ID numbers
  • Passwords or security credentials

Allowed Fields: Forms should only collect basic, low-risk contact details such as First/Last Name, Email Address, Phone Number, and general text comments or inquiries.

Getting Started with Your First Form

Access Permissions Required: Before you can create or edit forms, your CMS user account must be added to the Forms user group. If you do not see the option to create a Form asset, please submit a support ticket to request access.

  1. Navigate to Content > Forms in Modern Campus CMS.
  2. Click New and enter a name for the form, then click Create.
  3. Drag in your required elements (e.g., Single-line Text, Email Address, Radio Buttons).
  4. Set your email notification rules under Form Settings.
  5. Save, launch the form, and insert it onto your page using the Insert Form () button in the WYSIWYG toolbar.

Layout & Containers: Forms have their own built-in layout containers. While you can place a form asset inside a WYSIWYG Content snippet, doing so is completely optional—the form will display seamlessly on its own without adding extra snippets.
